The Clinical Use Of Improved Acupuncture Anesthesia In Abdominal Operations

454 subtotal gastrectomies, among 1335 abdominal operations under acupuncture anesthesia, were used to illustrate the promotion of the anesthetic effects by improvement of the selection of the stimulating parameters, selection of the needling points, and modification of the operative techniques as well as anesthetic acupuncture apparatus. Hyosine 0.3 mg, or morphine 0.5 mg i.m., and dolantin 1 mg/kg by rapid intravenous instillation were used as premedication. Juwei, right Zhangmen; Weishu, left Zhangmen (at frequency of 25-40c./min., current strength 0.4-0.6 mA); Qimai (bilateral, frquency of 30-60c./min, current strength 0.4-0.7 mA) were the points used, and their impulse duration is 1-1.2 msec.. The operative techniques should be modified accordingly, and a small upper abdominal median incision was made, the skin incision should be longer than that of the subcultaneous tissues and fascia. During exploration, the jejunum should be identified first. The anesthetic results obtained in subtotal gastrectomy was 91.1% with grade I and II. The authors emphasized the importance of the specificity of the acupuncture points. Pain thresholds and the stimulating parameters of each patient should be tested before operation. The frequency, strength of the current and impluse duration should be adjusted during the course of the operation in order to have desired analgesic effects. Finally acupuncture anesthetic apparatus used should have larger power output, and lower resistances, so as to give better analgesic results.
