1. Between 1970-1977, surgical treatment of 354 cases of urolithiasis were performed under acupuncture anesthesia. The total effective rate was 90.4%, including 328 cases of nephrolithotomy and renal pelvolithotomy, and 26 cases of uretrolithotomy.
2. Selection of acupuncture points and intercostal nerve blocking.
a. T[9-10] and L[2-3], 1.5 cm. Lateral from spinal process (ipsilateral).
b. Paraincisional points, one above and one below the incision line.
c. Intercostal block of T[10-11] space or T[11-12] space nerve root (1% xylocaine and 0.15% dicaine mixture) with 2 c.c. of the solution.
3. Some experiences. Extensive adhesion, large and multiple stones, nervousness etc. are contra-indications for acupuncture anesthesia. Using high stimulating frequency at the points above and below the incision line gave rise to a better analgesic effect, but at the same time, the muscle tone was highly increased. After the skin and the muscles were incised the electro-stimulation was stopped, the muscles then relaxed.
Due to imperfect analgesic effect of acupuncture anesthesia, intercostal nerve block enhanced its analgesic effect. Frequently changing the stimulating frequency also improved the analgesic effect.
As to surgery, less manipulation, light handling of the organ, shorter operating time were also contributary to the success of the operation.
