The Experiences In Abdominal Ligation Of Oviduct Under Acupuncture Anesthesia

Gynecology and Obstetrics Hospital, Congqing

2,047 cases, 90% of the trans-abdominal tubal ligation performed under acupuncture anesthesia were reported. Acupuncture anesthesia provides clear anatomical relationship, less bleeding, satisfactory muscle relaxation and better operation conditions. Acupuncture anesthesia is simple, safe, easy to master, with less complications and rapid recoveries. In this series, the anesthetists and operators were not fixed, they included house staffs as well as interns, yet the anesthetic effects were still good, with 96.7% success rate and 92.8% excellent and good results. Acupuncture anesthesia may be used as one of the routine anesthesias for trans-abdominal tubal ligation.

The relationship between the effect of acupuncture anesthesia and the points selected for needling, requisite of stimulation, duration of operation, and individual tolerance to needling were analyzed and discussed. We have the following impressions.

1. Good anesthetic effect could be obtained with either ear, lip or body needling. That is to say, we can use many needling points for one operation. The points most frequently used were ear needling or Gongsun point plus paraincision needling.

2. As to the requisite of stimulation, alternating sparse and dense wave was better than continuous wave.

3. The duration of operation is closely related to the effect of anesthesia. Short duration of operation is one of the important keys to anesthetic success. In all operations finished within 15 minutes the result was excellent. The duration of operation largely depended upon the smoothness in opening the peritoneum and lifting up the Fallopian tubes.

4. There is a great individual variation in anesthetic effects. It is easy to perform preoperative pain tolerance test simply by inserting one needle into the Hegu point. The effect of high pain tolerance patient was better with 95.8% success rate and excellent result in 74.1%, while those with low pain tolerance had excellent result only in 37.5%. Thus, we could predict the anesthetic effect before the operation. The cooperation and understanding among anesthetists, operators and patients are important in obtaining better anesthetic effect. During skin incision, exploration, lifting up of tubes and suturing of skin, gentleness is emphasized.
