The Phenomenon Of Arrival Of Propagated Sensation Along Channels To The Affected Areas-??Report Of One Case Of Functional Disturbance Of Mandibular Joints

Patient aged 22 years, male, a soldier, complained of pain and click in both mandibular joints when he opened mouth for 2 ½ years. The symptoms became worse after exposing to cold or heat. Various treatments such as, nerve block, physiotherapy and spasmolysis of bilateral muscli pterygoideus externus were ineffective. A few days before our accept, another operation was prepared for the joints again, during the induction of acupuncture anesthesia, the patient felt some sensation propagated from Gorgsun and Hegu, at which stimulation had been applied, along their own channels to the affected area. The pain subsided and symptoms markedly improved as soon as the sensation reached, thus the operation was cancelled. The patient was then referred to the 262 hospital of PLA for acupuncture treatment. After admission, the diagnosis was made as bilateral masseter muscle spasm. The patient was very sensitive to needling as he sweated promptly and the needling reaction could be got even with shallow puncture. Points which could develop PSC reaching the affected area along their own channels are Zusanli, Shangjuxu, Xiajuxu, Fenglong, Xuanzhong, Guangming, Yanglingquan, Fuyang, Gongsun, Hegu, Zhongzhu, Waiguan, etc. The time for PSC from the needling site to the affected area was about 3 minutes for the upper limbs and 5 minutes for the lower respectively. As soon as the PSC reached the affected area the pain disappeared, the mouth could be opened widely and mastication was to be much improved. Along the course of PSC there was a hypoalgesic zone like a strip where the degree of increase of pain threshold and its maintain time were in certain extent proportional to the intensity of the stimulation. Mechanical pressing or deep  injection of novocaine could block the PSC, but the effect of deep injection of normal saline was unsure. Electromyogram of the masseter muscles supported the above findings. The patient was basically cured after acupuncture treatment for 3 months and its effect was rather stable as followed up one month after discharge.
