1. Observation of curative effect in clinic:
(1) 209 cases were clinically diagnosed as cerebral thrombosis in convalescence, and some patients in acute period were diagnosed by means of lumbar puncture. The majority of the patients asked for acupuncture treatment when they found no obvious improvement after taking Chinese and western drugs. According to the course of the channels, Jianyu (L.I.15), Quchi (L.I.11), Hegu (L.I.4), Huantiao (G.B.30), Zusanli (St. 36), Juegu (G.B. 39) and Baxie (Extra. 28) on the affected side were used. For some patients the points on the head such as Baihui (Du. 20), Zhengying (G.B.17) and Xuanlu (G.B.5) were needled, for which the needle was rapidly rotated by hand or connected with pulse current. For the remaining points, the even re-enforcing and even reducing manipulation methods were used for 15 minutes each time. Ten treatments were considered as one course.
(2) Among 209 cases treated by acupuncture, 34 cases (16.3%) basically recovered, 49 cases (23.5%) greatly improved,99 cases (47.3%) improved and 27 cases failed. The total effective rate was 87.1% and the markedly effective rate, 39.8%.
(3) 209 cases were divided at random into 2 groups: the simple acupuncture group and acupuncture combined with drugs group. The former had 66 cases, in which the markedly effective rate was 30.3% and the gross effective rate ,83.3%, while the latter had 143 cases, in which the markedly effective rate was 44.1% and the gross effective rate was 88.8%. Contrasting these two groups, there is no significant difference in curative effect (P>0.05).
(4) Among 209 cases, there were 124 cases whose duration of illness was under 3 months and 85 cases over 3 months. For the former, the markedly effective rate was 51.6%, and the gross effective rate 94.4%, while for the latter, the markedly effective rate was 22.4%, and the gross effective rate, 76.5%. Contrasting the two groups, there is a significant difference (P<0.001). It demonstrates that the curative effect of acupuncture is better for the patients who have the duration of disease under 3 months than those who have the duration of disease more than 3 months.
2. Observation of objective index:
(1) Rheoencephalography (REG):
Observations were made for 21 cases of cerebrovascular convalescents and 10 cases of normal subjects, to whom acupuncture was given at the points of the head. The test results demonstrated that needling of the points of the head had no marked effects upon the cerebral hemodynamics of the normal subjects. But the retaining of the needle will make the arterioles dilate and speed up the outflow of the blood (outflowing time P<0.02). After withdrawal of the needle, this effect will disappear. The effects of acupuncture at the head points upon REG of the hemiplegic patients were greater than on that of the normal subjects. During the period of retaining the needle, the cerebral blood flow of the hemiplegic patients was increased and the peripheral resistance was decreased (the relative height of the second peak in REG, P<0.001). However, within 5 minutes of withdrawal of the needle, the small vessel resistance was increased (the index of the vascular resistance, P<0.01; the inflow speed, P<0.05).
(2) Amplitude of electromyogram (EMG):
Electromyographic observations were made in 24 cases of normal subjects and 27 cases of convalescents of cerebral thrombosis for 29 times and 51 times respectively. Acupuncture was given at the points of the head, or Futu (L.I.18) and Tianzhu (U.B.10) on the affected side.
The results of the test demonstrated that needling the points of the head can raise EMG amplitude of the normal subjects (P<0.05),>0.05). The rise of EMG amplitude of the patients after being electrified at the points of the head manifested at 5 minutes after acupuncture (P<0.05). Acupuncture at the points of the body may raise the EMG amplitude of the patients (P<0.05), the rise starting from 5 minutes after acupuncture till 45 minutes after acupuncture.
The above-mentioned results demonstrate that needling the points of the head may markedly and continuously raise the motor function of the normal subjects, but this effect is not significant in the patients. However, the points on the extremities and the head after being electrified may raise the motor function of the hemiplegic patient, and promote the recovery of muscular strength.
