The Summary Of 9,375 Cases Of Thyroidectomy Under Acupuncture Anesthesia

Until 1975 acupuncture anesthesia has been used in 8171 cases of different types of thyroid operations, and in 1976, 1204 cases of thyroid operations for hyperthy-roidism were performed. The success rate and the rate of grade I and II for the former group were 95.4% and 84.8% and for the latter group 95.28% and 82.67% respectively.

Methods of Acupuncture Anesthesia: Points and the frequency of stimulation used were Hegu-Neiguan 120-400 cycles/min; Futu and ear acupuncture 2400-5000 cycles/min. Continuous wave was used. The intensity of stimulation was gradually increased to the extent that the patient could tolerate. The induction time was 15-30 minutes.


Adjuvant drugs: In most cases phenobarbital was given 1 hour before operation and in some patients supplemented with valium or wintermine. Dolantin 1 mg./kg. was administered intravenously 15 min. prior to the operation, and, if necessary, another dose of 25 mg. added 2 hours later. Droperidol or valium and novocain were administered if necessary.

Factors influencing the effectiveness of acupuncture anesthesia: (1) Points: The rates of excellent and good results in 3 recipes of points were 85.5%, 83.8% and 85.7% respectively. The differences were not statistically significant. (2) The effectiveness of acupuncture analgesia was less satisfactory in complicated operations with severe trauma. (3) Duration of operation: The longer the duration of the operation the less satisfactory was the effect of acupuncture anethesia.


Selection of Points: 22 groups of different points were used for acupuncture anesthesia. Best results were obtained with 3 groups i.e. ear-needling, Hegu-Neiguan and Futu and their rates of grade I and II were, 87.38%, 85.02%, and 91.4% respectively. But other groups of acupuncture points also gave satisfactory results. The points had only relative specificity.

Indication & Contraindication: Acupuncture anesthesia for thyroid operations was effective, safe and reliable. Its rate of excellent and good results for various types of operations amounted to over 80%. It may be used in all patients except the uncooperative ones. If pre-operative preparations were satisfactory, acupuncture anesthesia could be used in most operations for hyperthyroidism. The preoperative requirements are very stable mental state, gaining of body weight, normal B.M.R. and pulse rate below 100. The beta-receptor inhibitory drugs such as practolol or inderal etc. helped to slow down the pulse rate and also helped to keep pulse stable during the operation. However the use of acupuncture anesthesia should be careful in patients with high B.M.R. and those having attacks of crisis recently.
